1. I authorize Choice International Consulting to submit my transcripts, certificates and related documents to applicable schools for credit transfer evaluation.

2. I acknowledge this CA$200.00 is for the credit transfer evaluation only. The fee will not be refundable.

3. I will provide all necessary information and documents required by Choice International Consulting for the credit transfer evaluation.

 

By electronically signing below and submitting payment, I confirm that I have read and understood the terms and conditions outlined above and agree to be bound by them.

 

These terms and conditions shall be governed by the laws of the province of British Columbia, Canada. You and Choice International Consulting agree to submit to the exclusive jurisdiction of the courts of British Columbia.
